Mail/Email Marketing List Estimate Request

NCBTMB mailing and e-mail lists are rented on a one-time use basis. A copy of the final marketing material(s) being mailed or emailed must be uploaded for approval before a list will be delivered. Marketing material must be exactly what you are sending out.

The lists will include the names and addresses or e-mail addresses of Board Certified Massage Therapists. An NCBTMB Board Certified Therapist reserves the right to forbid public release of any information obtained by NCBTMB.

Please note that lists sold by NCBTMB have not been processed through the National Change of Address (NCOA) database. NCOA processing is required by the United States Post Office in order to obtain postal discounts. NCOA processing will be the responsibility of the individual purchasing the list.

The e-mail and mailing addresses provided are the most up-to-date information that has been provided in our system. NCBTMB is not responsible for returned mailers due to changes in address. NCBTMB is also not responsible for e-mail open rates, spam, unsubscribe actions, etc. due to changes in e-mail address or individual’s wishes. NCBTMB will not include individuals who have opted out of 3rd party mailings.

I understand and agree that if I proceed with an order after receiving the estimate that this list order is for a one-time use and is to be used only to send material submitted for review on the date of the mailing specified above and approved by NCBTMB. Neither the list nor excerpts contained are to be duplicated, reproduced, reused or transferred without written authorization. A separate order form must be submitted and approved for each mailing and list use. I understand that list orders are seeded with decoy names to detect unauthorized use. If unauthorized use is detected, I understand that I will incur additional charges, up to 10 times the value of the original list order and may be subject to additional legal action. Lists must be used within 2 months of date of receipt.
