Why Choose Board Certification

In consideration for value received, I hereby irrevocably grant to the National Certification Board for Therapeutic Massage & Bodywork, its affiliates, agents, and assigns (NCBTMB) permission to use, publish, copyright, and incorporate (alone or with other materials) photographs and/or video footage taken of me, my name, my statements/quotes, and/or other identifying information in NCBTMB’s marketing, educational, and communication materials (including but not limited to brochures, magazines, newsletters, advertisements, internet, posters, video, TV, or any other media), without my approval or further compensation of any kind to me. I understand that NCBTMB may choose not to use my likeness or information or may choose to use my likeness with or without using my name and to make alterations and/or additions thereto for NCBTMB’s use. I hereby release NCBTMB from any claims, liabilities, and expenses that I may have in connection with NCBTMB’s use of my likeness, name, quotes, and/or information. This agreement fully represents all terms and considerations and no other statements, conditions, or promises have been made in conjunction with this release form.
